RHP2 Readmission Learning Collaborative Quarterly Progress Update

Sir/Ma’am,

At our last scheduled face to face learning collaborative meeting (August 15th), we discussed the next steps for the RHP-2 Readmission Collaborative. The UTMB Anchor office will continue to monitor the progress on your projects and what’s going to be useful in order to help us sustain improvements on the RHP-2 readmission results. Please complete the [attached] brief interval progress update on your projects quarterly; this is intended to monitor how we as a region are sustaining our gains. At your convenience, please complete the progress report update template and email it back to me by COB November 7th. Attached is also a [completed sample template] for your use. Reports will be posted prior to the next conference call so that we can enhance the dialogue. The next Readmission Learning Collaborative quarterly status update conference call on data collection is scheduled on November 14th, 2014. Please let me know if you have any questions.

RHP 2 Readmission Collaborative Conf Call/Webinar – September 26th

Attached is RHP 2 Readmission Collaborative conference call/webinar [notes]. The PowerPoint presentation (large file) and additional info will be posted to the RHP2 blogs link below by the end of the week [attachment 1] [attachment 2]. This was the last bi-weekly conference call/webinar. Starting October 1st, 2014, Readmission Collaborative members will be required to submit their DSRIP project data quarterly. The next Readmission Learning Collaborative quarterly status updates conference call and data collection is scheduled on November 14th, 2014 at 1:00 PM CST. Please let me know if you have any questions and thanks for those Collaborative members that participated in the call/webinar.
