Behavioral Learning Collaborative Event Friday, January 15, 2016 – Lufkin

We look forward to seeing everyone at Burke in Lufkin as we hold the first DY5 in-person Behavioral Learning Collaborative event. The meeting is set for 10:00 – 2:00, and the plan is to focus on two major topics: What Does Transitions of Care Mean in Behavioral Health and Waiver 2.0.

The address is: Burke, 1522 West Frank Avenue, Lufkin TX 75904

The team at Burke have provided the following information regarding directions.

  • From Houston, take 59N to Lufkin.  When you enter Lufkin, turn left onto Loop 287.  Continue on Loop 287 approximately 3 miles.  Exit Hwy 94/Frank St.  Turn right onto Frank St.  In approximately 0.6 miles, Burke is on your left.
  • From Beaumont, take 96N to 287N to Woodville.  Take 69N from Woodville to Lufkin.  When you enter Lufkin, turn left onto Loop 287.  Continue on Loop 287 approximately 7.2 miles.  Exit Hwy 94/Frank St.  Turn right onto Frank St.  In approximately 0.6 miles, Burke is on your left.
  • When you arrive at Burke, follow the driveway to the back of the building.  Enter the building through the main (only) back door.  The first hallway to the right once you enter the building will take you to the training room.  There will be signage to follow once you enter the building.

There will be a Readmissions Learning Collaborative conference call on January 7th, 2015 at 9:00 AM CST (see agenda).  Collaborative members are reminded to provide their DSRIP project quarterly status update during these calls.  Please complete the attached quarterly project progress update template and return completed form to me by COB January 5th, 2016. Additionally, please provide your organization’s 30-day all cause readmission rates data for Q3/Q4 FY 2015 on the same form.
