As part of the University of Texas System Population Health Improvement Initiative, you are invited to participate in a research project.  This is a stakeholder survey to help us identify the critical needs of your community.

The survey will take about 15 minutes to complete.  Your completion of this survey indicates your consent to participate.

You will be asked to answer questions about issues in your community such as access to services and health and environmental problems. Your responses will be used to prioritize health concerns in your community.

7/22/16 Anchor Notes

Good morning Region 2,

Please see the attached for the information received after the Anchor call with HHSC last Friday. Please note that:

  • HHSC has provided a draft copy of the Statewide Learning Collaborative agenda.  If your organization plans to send representation but have not yet registered, the cut-off date is August 8th.
  • From the Anchor notes:
    • HHSC will be uploading an amended copy of the Category 3 summary workbook, which will reflect some changes in DY5 & DY6:
      • Targets for selected outcomes under Outcome Domains 10 & 11.
      • New Category 3 IDs have been issued for projects being transferred over from other Regions.
    • HHSC will issue feedback to the Anchors beginning August 26th on the DY6 participation templates; if there are any NMIs from that, our office will coordinate with the appropriate provider(s).
    • HHSC is holding a public hearing in Austin on August 26th on DY6 Rule Packet #2 (webcast will be available).

Also attached is a draft SLC agenda so that you can see the general topic areas.  If you share this with providers, please make sure they understand it is a draft and subject to change.  We will send you an updated SLC registration summary early next week.

I’ve also attached a map that shows the Medicaid managed care Service Delivery Areas overlaid with the waiver RHPs, which you and your providers might find useful. We will get it posted soon on the waiver website.

Regarding the updated regional community needs assessment, the PFM states it will be due in June 2017 in order for an anchor to receive the DY6A Anchoring Entity allocation.

HHSC will be making the final DY 5 Uncompensated Care (UC) payments to all eligible hospitals in September 2016. There are several important upcoming deadlines which pertain to UC payments that providers should be aware of in the table below. Please note that if your TPI has changed between DY 4 to DY 5 you will need to update it with the anchor of your RHP and the Transformation Waiver Team.

7/8/16 Anchor Update

Good morning,

Thank you to everyone who joined us for the Regional Meeting in Galveston; we hope it was beneficial for your ongoing DSRIP efforts.

Please see the brief Anchor update that was issued by HHSC late on Friday, as well as the attachments.  The draft DY6 Core Components questions are now open for review and feedback (due to HHSC by July 22nd); we ask that you copy Jennifer Woodard (jnwoodar@utmb.edu) and/or myself (smseiden@utmb.edu) if you decide to submit feedback.

Also attached is the list of registrants currently showing for the Statewide Learning Collaborative, which will be August 30 & 31 in Austin.  We do have providers looking for additional spots so for those organizations who will not be sending anyone to Austin, please let Bobbie Guyton (bjguyton@utmb.edu) in our office now ASAP.  HHSC has set a registration deadline of August 8th.

A separate email will be coming from me shortly with the Category 3 information referenced below.

Dear Anchors,

Here are a few updates:

DY6

  • The attached draft questions for core component reporting milestone reporting were posted yesterday on the waiver website for provider feedback. Please submit any questions or feedback regarding these draft questions to TXHealthcareTransformation@hhsc.state.tx.us by July 22, 2016, and include the subject line: “Core Component Milestone Reporting Question Feedback.”
  • We posted an updated DSRIP Participation Template yesterday, which corrects an issue with calculating IGT for Category 3 outcomes on theDY6 IGT Information  If you have already completed the previous template, the updated template does not need to be used because the change does not impact provider entries.

Statewide Learning Collaborative Summit

  • Please see the attached list of registrants for the 2016 Statewide Learning Collaborative.  The list is organized by RHP and performing provider, including details for the total number of slots allocated per provider (i.e., TPI) as well as the number and name of registrants as of June 28, 2016. As previously described, performing providers that cross regional lines have been assigned to a single RHP in the attached file and were allocated slots based on the total number of projects affiliated with the provider’s TPI.  We have included a reference in the file to guide these providers to the RHP they are assigned to for the purposes of registration.
  • Please work with your regional providers that have not yet registered for the event, noting the deadline to register of August 8th. After this date, HHSC will be redistributing any remaining slots.  HHSC staff will continue to confirm panelist presenters and identify opportunities for additional slots for these individuals.

To be guaranteed a spot at the conference, designated individuals must register at https://2016slcsummit.eventbrite.com by Monday, August 8th. The registration passcode is HHSC2016.  Upon completing registration, registrants will receive an e-mail confirming their registration.

Category 3

  • An updated Category 3 Summary Workbook & Goal Calculator and the Category 3 Interim Correction Template for DY5 R2 should be posted this afternoon on the waiver website.  We will send those links to providers and anchors when they are posted, along with some additional guidance for Cat 3 in DY6.
  • The updated Category 3 RHP Summary will be available next week.

DSRIP Category 3 Update – DY6 Operational Details, Updated Category 3 Summary Workbook, & Interim Correction Template Available

Please see the guidance below on the DY6 Operational Details for Category 3.

Additionally, please note that the latest Interim Correction window for Category 3 is now open.  For your convenience, I’m attaching the correction template and the reporting summary/goal calculator files (which, as stated below, solidifies *most* of the DY6/PY3 targets) below.  If your organization needs to submit a correction, it is due back to the HHSC mailbox by COB on Friday, July 29th.

DSRIP Anchors,

The following documents related to Category 3 are now available, and have been sent to providers:

  1. Category Operational Details for DY6 (Attached):
  • This document is an outline of operational details for Category 3 in DY6, intended to provide additional clarification on Category 3 operations prior to finalizing the second rule packet and RHP Planning Protocol for the Transition Year. It contains a general update on Category 3 in DY6, and additional explanation of allowable measurement periods, goal calculation and partial payment.

 

  1. Updated Category 3 Summary Workbook and Goal Calculator (http://www.hhsc.state.tx.us/1115-Waiver-Guideline.shtml Category 3 section, middle of the page):
  • The Category 3 Summary Workbook and Goal Calculator allows providers to review outcome selection details and reporting history at an individual outcome level, as well as calculate achievement and goals if corrections are anticipated. The Category 3 Summary Workbook has received the following updates from the prior version:
    • The “Category 3 Summary” tab has been updated to include April DY5 reporting, MSLC baseline reviews reported to HHSC as complete by 05/31/16, and DY6 goals for most outcomes. DY6 goals for IOS – Survey outcomes using Scenario 2 or Scenario 3 will show as “TBD” pending a final goal setting methodology. Interim correction eligibility, and anticipated October reporting eligibility are also included.
    • The “Goal Calculator” tab now includes DY6 goals and achievement calculations, following the methodology approved in the Transition Year section of the Program Funding and Mechanics (PFM) Protocol.
    • The “Goal Calculator” tab can now be used for both primary category 3 outcomes and Population Focused Priority Measure (PFPM) outcomes. In cases where a provider has a PFPM selected, the goal calculator tab will be applicable to the PFPM outcome only.
  • Providers should review the content of the Category 3 Summary Workbook for accuracy and contact HHSC if there are any errors identified in reporting history, measure selection, measurement periods, milestone structure, approved subsets, and goal calculations. If previously submitted baseline or performance rates are incorrect due to errors in data abstraction or measure specification understanding, providers can submit a correction through the interim correction period if eligible, the October DY5 reporting template if eligible, or through MSLC if applicable.

 

  1. DY5 R2 Category 3 Interim Correction Template (http://www.hhsc.state.tx.us/1115-Waiver-Guideline.shtml Category 3 section, middle of the page):
  • The Category 3 Interim Correction Template is for eligible outcomes that need to submit a correction to prior reporting.
    • As a reminder, the following outcomes are eligible to submit a correction during the interim correction period.
      • P4P outcomes that have previously reported performance and are not under performance review by MSLC. Please note that outcomes with a baseline previously corrected by MSLC may be eligible to submit an interim correction for performance reporting only.
      • Outcomes approved for a custom goal calculation or milestone structure (maintenance, P4P change to P4R due to small volume, HHSC approved alternate achievement request)
    • Providers should complete one template for each eligible outcome needing a correction.
    • Interim correction templates should be submitted to the waiver mailbox by Friday, July 29th.

RHP 2 — Learning Collaborative survey link (please complete by July 22nd)

Good morning,

As I mentioned during the Regional meeting last week, we are asking for feedback from all of our Performing Providers on the Learning Collaborative structure (including what you’d like to see us focus on in the future).

7/1/16 Anchor update

Dear Region 2:

It is not too late for you to RSVP for this Friday’s Regional meeting in Galveston.  The event is Friday, July 8th, and will be held at UTMB’s main campus.  Please note that HHSC Waiver leadership will be attending and updating Region 2 performing providers and stakeholders on the current waiver renewal negotiations with CMS.  Equally important is the discussion that Dr. Laura Rudkin, professor & chair, will lead with the assistance of Dr. Mark Kirschbaum, chief quality safety & clinical information officer.  Drs. Rudkin and Kirschbaum will discuss the Regional Community Needs (re)assessment.  This is a critical part of our regions strategic planning and necessary for considering possible replacement projects and/or project modifications going into Waiver 2.0.

As of today, we have well over 100 attendees registered, so if you have not yet responded, please RSVP your attendance to Bobbie Guyton at bjguyton@utmb.edu.

Please see below for the short update received from HHSC on DY6, and the new information that one (1) of the four (4) Category 1/2 deliverables will be eligible to be reported in the April 2017 cycle.

Hi, Anchors –

We  wanted to send you a few updates:

  • We haven’t forgotten your request last week for the breakout of SLC registrants per region and we’ll get you that as soon as possible.
  • Anchors will have until December 15, 2016, to submit the regional learning collaborative plan for DY6A.
  • Several anchors and providers have asked if the non-QPI milestones will be eligible for reporting in April 2017.  HHSC does not plan to make either of the two DY6A non-QPI milestones eligible for reporting in April 2017.  However, if a project has an MLIU QPI milestone that is P4R for DY6A, the provider may report the project’s MLIU QPI achievement for the first half of DY6A during the first reporting period for DY6A (April 2017), regardless of whether the provider has achieved the project’s MLIU QPI goal for DY6A. Upon HHSC review and approval of the MLIU QPI milestone, the provider would receive payment for the milestone.  Note that the provider will also need to report the project’s MLIU QPI achievement for all of DY6A during the second reporting period for DY6A (October 2017) as part of semi-annual reporting (SAR).
