This summit has been designed to offer an opportunity for providers at all levels, including those working outside of DSRIP, to come together and discuss strategies to strengthen the quality of care provided to patients. We expect participation from a wide variety of regional constituents, including administrative and clinical leaders and front-line staff from hospitals, nursing homes, rehabilitation facilities, skilled nursing facilities, and home health agencies. We also encourage participation from our behavioral healthcare providers and community physician partners, as we all contribute to the concept of “whole person wellness.”

To register for the event, please visit: Registration on/before Monday, March 20, 2017 will assist with final preparations, including catering requirements. Please note that efforts are in progress to potentially offer continuing nursing education (CNE) hours for attendees in conjunction with UTMB’s School of Nursing.  If you have additional questions, please contact a member of UTMB’s Care Transitions Summit planning team: Susan Seidensticker (, Fred Lopez ( or Michael Leger (

This cross-organizational outreach has the potential to truly move the needle on readmissions; so, I hope you accept our invitation to participate on March 24th.
