Registration Open for The 2017 Care Transitions Summit, Hosted by UTMB on Friday, March 24, 2017

Good Afternoon RHP 2 Providers, Regional Stakeholders, and Statewide Partners:

As part of our ongoing anchor role, leadership of both Region 2’s 1115 Waiver Office and The University of Texas Medical Branch at Galveston (UTMB Health) have established a strong commitment towards identifying and sharing best practices related to readmissions. To this end, we would like to invite you (and others from your organization) to attend our upcoming learning collaborative event. The 2017 Care Transitions Summit will be held on UTMB’s Main Campus in Galveston on Friday, March 24, 2017 from 10:00 a.m. to 4:30 p.m., with a working lunch included. Additional details, including meeting location and parking instructions, will be provided to registrants in advance of the event.

This summit has been designed to offer an opportunity for providers at all levels, including those working outside of DSRIP, to come together and discuss strategies to strengthen the quality of care provided to patients. We expect participation from a wide variety of regional constituents, including administrative and clinical leaders and front-line staff from hospitals, nursing homes, rehabilitation facilities, skilled nursing facilities, and home health agencies. We also encourage participation from our behavioral healthcare providers and community physician partners, as we all contribute to the concept of “whole person wellness.”

Please note that efforts are in progress to potentially offer continuing nursing education (CNE) hours for attendees in conjunction with UTMB’s School of Nursing. Region 2 is operating multiple Learning Collaboratives.  Membership is open to all performing providers in the region, as well as other interested stakeholders who would like to participate.  The collaboratives strive to be transparent, and encourage broad-based participation in an effort to foster an active learning environment within the 16-county region.  The initial topics focus on Preventable 30-Day Readmissions and Behavioral Health.
